Happened today in Fremont, Nebraska.
The plant reportedly made wood pellets. This just underlines the how serious woodshop dust control for DMS is.

Father worked for Gerogia Pacific when the Corrigan plant experienced a dust collection system explosion after which two people subsequently died. While he was not directly involved, the entire company got very very serious about plant safety well before the subsequent civil suit and resulting judegement.
