And I am stilling waiting for two, One the form was there, the second one I just emailed the infro. There were for the inkle classes
I am assuming the form you want is to put in the info after the class is taught. THIS should be that form.

I know another person that just taught a class that needs this too. I’ll give her this link, but probably others are in same boat of recent classes taught under old system that need to claim honorariums.
