So I appologize - I sent out a non-working form to everyone and asked people to start entering data.
I went in and fixed it and I had the wrong link posted (thank you guys!)
The section for Entry #2 does not follow the same format as sections for Entries #1 and #3, and makes references to the third entry instead of the second.
Specifically:
Write a brief explanation of your third work [should be second]
My third entry falls under which committee? [question is repeated, and refers to Entry #3]
How should your second piece be displayed? [out of order compared to sections for Entry #1 and #3]
